Official Rules for Refresh You “Win an Invitation to A Night with Chef Rod” Contest

Description of Contest – Dean Health Systems, Inc. (“Dean”) is offering the “Win an Invitation to A Night with Chef Rod” contest and will award the prize via a random selection of valid entrants.  The contest will begin at 12:01 AM July 1, 2011 Central Daylight Time, and ends July 14, 2011, at 11:59 PM Central Standard Time.  The drawing will be conducted within one day of the entry deadline.  The winner will be notified upon being selected.

1. Eligibility – This contest is open individuals at least 18 years of age and is limited to one electronic entry per individual.  To be eligible, entries must be made by an individual from a single computer and may not be made by proxy.  If the winner is deemed ineligible, another ticket will be pulled and so on until an eligible winner is chosen.  The winner will be contacted directly by Dean.  Valid entry submission allows consent to post contest winner’s name.  The directors, officers and employees of Dean, its subsidiaries or affiliates, as well as their spouses and immediate household members are not eligible to participate.  Dean reserves the right to verify eligibility qualifications of any winner.

2. Prize – Winners will receive up to four invitations to the event, held Tuesday, July 26, from 6:30 p.m. to 8:30 p.m. at the Mansion Hill Inn in Madison, Wisconsin. Up to 60 invitations will be randomly awarded.  Odds of winning depend upon the number of valid entries received in each drawing period.  No substitution of prizes or cash will be accommodated or permitted.  Dean is not responsible for any late or misdirected award responses.  No partial acceptance, transfer of prizes or cash redemptions from unused portions of the Prize will be permitted. Sponsor reserves the right to substitute a prize of equal or comparable value in the event a described prize or any portion thereof is unavailable for any reason whatsoever.  Winner is solely responsible for the reporting and payment of all federal, state and local taxes incurred by acceptance and use of the prize (or any portion thereof), if applicable.  Prizes that remain unclaimed after 90 days will not be awarded.

3. Release. By entering the contest, you release and hold Dean and its employees, officers, directors, members, shareholders, and marketing agents harmless from and against any and all losses, damages, claims, and actions of any kind arising in connection with the contest or resulting from acceptance, possession, use or misuse of, or defects in, a prize, or participation in the contest, including without limitation claims based on personal injury, death, property loss or damage, publicity or privacy rights, and/or defamation.

4. General. Participants and winners agree as a condition of participating that the laws of the State of Wisconsin shall govern this Contest. If in Dean’s sole opinion, there is any evidence of Contest tampering, any violation of these Official Contest Rules, or if technical difficulties compromise the integrity of the Contest, Dean reserves the right to declare entries invalid or terminate the Contest and conduct a random drawing to award the prizes, using all valid entries received up to the date/time of corruption or modification, to the extent feasible.  Official rules for drawing may be amended by Dean at any time.  The decision of Dean shall be final on all matters concerning the drawing.

Official Rules for Refresh You “Win Dinner for Six, Prepared by Chef Rod, in Your Home” Contest

Description of Contest – Dean Health Systems, Inc. (“Dean”) is offering the “Win Dinner for Six, Prepared by Chef Rod, in Your Home” contest and will award the prize via a random selection of valid entrants in attendance, by invitation only, at the A Night with Chef Rod event on July 26, 2011 from 6:30 p.m. to 8:30 p.m. at the Mansion Hill Inn in Madison, Wisconsin .  The contest will begin at approximately 8:30 p.m. on July 26, 2011 Central Daylight Time, and ends shortly thereafter with the random drawing of a single winner.  The winner must be present to win.

Eligibility – This contest is open individuals at least 18 years of age who have won invitations to the A Night With Chef Rod event.  If the winner is deemed ineligible, another ticket will be pulled and so on until an eligible winner is chosen.  The winner will be contacted directly by Dean.  Valid entry submission allows consent to post contest winner’s name.  The directors, officers and employees of Dean, its subsidiaries or affiliates, as well as their spouses and immediate household members are not eligible to participate.  Dean reserves the right to verify eligibility qualifications of any winner.

Chef Rod will provide all ingredients and cooking equipment necessary to prepare the meal. The exact menu and food selection will be determined by Chef Rod.  Odds of winning depend upon the number of valid entries received.  No substitution of prizes or cash will be accommodated or permitted.  Dean is not responsible for any late or misdirected award responses.  No partial acceptance, transfer of prizes or cash redemptions from unused portions of the Prize will be permitted.
